OSHA Express section is a work-in-progress. What is ALREADY here should be correct.
Inspection Tab
- Optional Report Number
- The format is ###-YY, where ### is the three-digit number matching this inspection's place in the sequence of your opened inspections for the fiscal year; and YY is the last two digits of the federal fiscal year in which you opened it.
- NAICS/SIC
- Some SICs will not work. These include SICs like 3000 "not elsewhere classified."
- Related Activity
- Complaint, Referral, etc., number.
- After opening inspection, check either “Sat Health?” or "Sat Safety?" and check “Cmp/Ref Close."
Dates Tab
- Only the date fields are required in the date/time sections. Do not enter any information into the time fields.
a. Advance Notice
- This should always be "No" unless you have particularly unusual circumstances.
b. Entry Date / d. Opening Conference Date / f. Walkaround Date
- These should be self-explanatory.
h. Final Closing Conference / j. Closing Conference Date 2 / l. Exit Date
- These should all be the same date, which is the date on which the last closing conference was held. (Per Mike Hutcherson as of 10/3/14.)
- If it was a quick/simple inspection and the final closing conference was on the same day as the opening, these dates can all match b., d., and f.
Emphasis/Initiatives Tab
- If any Local or National Emphasis Programs or Strategic Plan Activities are related to your inspection, enter that information here.
- You can click "Add" beside one of those sections to get examples. Just remember to either delete that record or not save the inspection when closing.

General Notes for All Narrative Sections
- Information and data does not have to be repeated if it’s already been included in the “Found” of another complaint item. Reference that other item and include any additional information that didn’t apply to the other item. (per Mike’s email 5/4/12)
- This would also apply for any other sections of the narrative. [NOTE: This does not apply to referencing anything written within a citation description.]

- Include the manufacturer and names of any chemicals referred to by the inspection. This list of materials can also go into the citation's hazard description

D. Opening Conference Notes
- Address items such as:
- When arrived, Where arrived, First Contact, Credentials to whom, Right to refuse, Scope and purpose of visit, What CSHO did at opening conference, What employer did, Who was in attendance, Employee & employer representative info (e.g., union), Employees’ rights to a discrimination free workplace, Unusual circumstances encountered

- Date/time arrived,
- People in attendance
- Content covered, which should include:
- PIG, Violations, General Severity of violation, No Discrimination toward employees who participated in investigation
- Any unusual circumstances encountered
G. Summary of Complaint, Referral, Accident or Followup Findings
- Complaint Item(s)
- Type complaint item word for word as received on complaint form—followed by what was Found in relation to this complaint. Usually, begin with describing the related process in general and then work in the specifics found about the process during the inspection.
- Format: Complaint Item #1: then the item; then Found: then the observations.
- Make sure to include whether a citation is or is not recommended. (per Mike’s email 5/4/12)
- Referral, Accident(?), and Follow-up items are treated in like manner as Complaint Items.
- Failures to Abate
- Format: Citation [#], Item [#]: [Previous AVD including {Standard}: {Standard text}: and items]; then Found:, then description of failed abatement.
- Photo Summary Sheet, including Non-Citation Related Photographs

Evaluation of Employer's Overall Safety and Health Program
- Address the following topics in a similar fashion to how the S&H programs were addressed in the late, great WordPerfect:
- Monitoring Programs (Noise, personal etc)
- Recordkeeping Program (OSHA-300, IA-1s, 1910.1020)
- Hazard Communication Program (HazCom)
- PPE Hazard Assessment Program
- Lock-out/Tag-out Program
- Evacuation Program/Emergency Response
- Confined Space Entry Program
- Fire Prevention Program
- Written Compliance Plans (Expanded Standards, PSM, etc)
- Other Programs
- For each program, make sure to address what is included and, if applicable, what is not included.

- Bulk or Wipe Samples
- Enter only “Substance Code,” “Requested,” and “Sample Type.” Mark if citation issued.
- Leave out “Exposure” items. Prevents saving.
